Douglas County Community Health Improvement Plan

The Community Health Improvement Plan (CHIP) is a strategic plan for health and wellbeing for a community. The Douglas County CHIP, which is a process that is undertaken every 5 years, is a roadmap for health that includes goals, objectives, and strategies for advancing equity and making meaningful progress towards health-related outcomes.

2024-2029 CHIP

The 2024-2029 Community Health Improvement Plan was presented to the public on October 17, 2024.

The development of this plan was guided by multiple partners and a steering committee, which provided leadership and oversight to the 2023 Community Health Assessment and planning process. The steering committee included representation from the county's four municipalities - Baldwin City, Eudora, Lawrence, and Lecompton. Nearly 20+ people served on the steering committee, representing government agencies, nonprofits, education, faith-based organizations, and businesses.

The plan is broken up in to six pillar categories that represent key issues of interest in our community that were identified in the Community Health Assessment (CHA). Each of the six pillars was represented by a convening group who selected areas of priority and objectives for improving health outcomes.
